We'd like to let you know that it is our practice policy that the fee payable for an initial consultation is payable at the time of booking. We can take payment for this by credit or debit card over the telephone.

If you are a new patient you will need to fill out and read the following relevant forms:

*To help you keep track of your appointment, we will send you an email 2 weeks prior, and a text reminder 5 days prior to your appointment*

Medical History Questionnaire (To be completed by all new patients)

Smile Check Questionnaire (To be completed by all new patients)

Nervous Patient Questionnaire(To be completed by all new patients who are nervous or anxious about their visit to us)
Orthodontic Questionnaire (To be completed by all patients enquiring about braces)
TMJ Questionnaire (To be completed by all patients suffering with headaches)
Ravenscourt Dental Privacy Notice (GDPR-EU Regulations)

To return the forms by post:

Download the relevant questionnaires
Print the documents and fill in your details
Send the completed documents to us in the post: Ravenscourt Dental Practice, 5 Ravenscourt Avenue, London, W6 OSL

To return the forms electronically:

Edit the document with your details
Save the document on your PC
Send an email to [email protected] with completed forms as attached files.
